First of all, let me give you a brief background of the anime. It was created by Naoki Urasawa

and it consists of 74 episodes.
It contains a blend of mystery, horror, psychological thriller, manipulation and a huge load of suspense. It's also set in the early 90s.

Episodes 1-10

The anime begins with a renowned Japanese neurosurgeon called Dr Tenzo Henma

who is known to be one of the best in his field. As a result of his expertise, he has a very bright future in the Eisler Memorial Hospital where he works in Germany. He is engaged to the owner's daughter called Eva Heinemann and this also gives him an edge over others.
One day he is given orders to operate on a government official instead of a patient that came in earlier, he does it and battles with his thoughts later. He then decides to talk to his fiance who tells him that ' all life is not equal'.
What she said stuck with him and he didn't agree with it in his mind as that goes against the basic ethics of the profession.

On one faithful day, a boy who sustained a bullet in his head was brought in along with his sis who was traumatised and the only thing she could say was 'kill him..'

. The mayor of the city was also brought In that day and Doctor Tenzo was asked to operate on the Mayor instead of the boy.
He then disobeys orders and saves the boy

and this leads to the death of the Mayor.
As a result of this, his fiance

broke up with him and the owner along with a few others turned against him.
On one occasion when he was ranting to the unconscious boy whose life he saved, Tenma said he wishes the Mayor and all of them would just die. He didn't know that the boy whose name was Johan heard him and the boy was a sociopath

.
The next day, a strange thing happened. The mayor along with a few others were found dead and the boy Johan Liebert and his sis Anna Liebert were nowhere to be found.
This left poor Doctor Tenma devastated a bit but alongside with that he was also promoted to Chief of Surgery which raised a few eyebrows.

9 years later after some subsequent series of murders, Doctor Tenma found out that the boy he saved 9 years ago was responsible for all of it. He found out when a patient of his who was also suspected of a crime that Johan

orchestrated escaped the hospital. Tenma then witnessed Johan murder the man in a reclusive area.
Johan then thanked Doctor Tenzo for saving his life 9 years ago. Tenzo was bewildered with shock and started to question his actions 9 years ago. Whether it would have been better if he just saved the Mayor instead of the boy.
Doctor Tenma has been the suspect in the series of murders.
Doctor Tenma then investigates the past of Johan Liebert and found out that Johan went to 511 Kinderheim orphanage home where the government led a secret experiment to create perfect soldiers. Doctor Tenma also finds out that Johan was the only one that survived the orphanage and that everyone else killed each other. He also finds out that Johan was responsible for the death of 50 people in the orphanage.
Doctor Tenma then gets very obsessed with stopping Johan and his evil deeds.

Episode 11-20

Nina Fortner

also regains her memory that she is Anna Liebert where she stays with her Foster parents. Johan also orchestrates the killing of her new Foster parents and Tenma comes to rescue her, she then remembers that she shot her brother-Johan in the head 9 years ago. And that it was Johan that requested it when she found out that Johan has been the one killing everyone and he killed their first Foster parents.

Doctor Tenma

becomes a fugitive in his search for Johan as he was accused of murder. On one of his expeditions in search of Johan and Johan's past. He meets a man who's called Mr Hartmann who once worked at 511 Kinderheigm orphanage home, Hartmann is taking care of a young boy who Tenma helped patch up his wounds on the street. He found out Hartmann maltreats the boy and that the man wants the boy to be like Johan. Doctor Tenma then comes to the boy's rescue and Hartmann tells Tenma about Johan's past and how Johan was the only one who survived. And when Hartmann asks Johan how he did it Johan just said he increased their hatred and when he also asked Johan his goal he said to watch everyone in the whole world kill each other and to be the last human alive. Dieter (the boy living with Hartmann)

is made to choose who to stay with and he chooses Tenma. Frustrating seeing this, Hartmann then wants to shoot Dietsr and Tenma brings out his gun too and the man doesn't shoot. Tenma then tells Dieter that he can't stay with him and that he wants to take him to an orphanage home but Dieter is persistent and sticks with Tenma.
A detective named Inspector Lunge

and also Tenma's ex-fiance called Eva Heinemann

is also obsessed with capturing Tenma although she knows that Tenma couldn't kill her father. She is just obsessed with having Tenma back as she says that's the only man that loved her.

Johan manipulates wealthy people and continues with his series of murders.

Episodes 21-30

An investigative reporter named Richard Bruan

investigates the death of Edmund Fahren(a guy who pretended to be the son of Hans Georg Schuwaldsuicide a wealthy man

) and he finds out that it wasn't a suicide. After his intense investigation, he finds out the person responsible for it was Johan and Johan was working for Hans Georg Schuwald at that time. Johan then finds out that Richard Braun had been investigating him all this while. He then manipulates Richard Braun and kills him

. The doctor in charge of Richard's drinking habit knew a bit about the case Richard was working on and that he was suspecting Johan was Dr Reichwein. Dr Reichwein and Rudy Gillen

who's a criminal psychologist- a former classmate of Tenma knew that Richard's death wasn't just an accident or a suicide but it was a murder. They started investigating it more and Johan sent people to kill Dr Reichwein once but it failed. The second time Johan sent the hitman who does most of the killing for him. The hitman disguises himself as a client and seeks counselling from the doctor and comes as the last patient when everyone else in the workplace has left. In the conversation with his supposed client,
Dr Reichwein

finds out that Johan sent the hitman to kill him and he then thinks of ways to escape. He finds out all the doors are locked and as the hitman is about to shoot the doctor, Tenma comes to his rescue.
